Add A Contact Form (Group Question Fields Into Form)#409

I have client feedback on the questions module for name, email, phone etc. They believe that it lowers conversion rate because the lead may not anticipate how many fields they have to fill out in succession.

Can we group generic questions like name, address, phone into a Contact From style field?

I would love to have this feature! :)
